The Provost Council voted and approved that the Office of Academic Assessment will only process the Electronic Course Evaluations (ECE) beginning with the Fall 2014 semester. The current paper-based course evaluation forms (Green Course Evaluation Form, and the SIR-II Course Evaluation Form) will no longer be ordered and used after the Summer 2014 Session. The questions on the Green Course Evaluation Form have been uploaded into the electronic format (now called the E-Green Form). Until further notice, the E-Green Form will be the standard form on the campus. Department-specific items can be added to the E-Green Form by contacting the Office of Academic Assessment.
