English as a Second Language Program Staff and Expenditures Report

The purpose of this report is to collect English as a Second Language (ESL) information from SAUs for use in updating the ESL weighting component in the Essential Programs and Services (EPS) funding model, as required by state legislation.

You are asked to provide information for the 2005-06 and 2006-07 fiscal years.  Note: Do not include expenditures from Title III accounts or Special Revenue Fund 268.

The report may be completed and submitted in two ways as described below.

Please complete and submit the completed report by October 29, 2007.

To fill out the report please either download the Excel version and complete the report electronically or download and print the pdf version and complete the report by hand.

Option 1 - Excel File

Instructions:

• Download the Excel version to your computer and fill in the information requested electronically on the spreadsheet.
• Save the document with your SAU name added to the document name.
• Either e-mail a copy of the completed report to Ida Batista at ibatista@usm.maine.edu, or print a copy of the completed report and mail it to: CEPARE, c/o Ida Batista, 99 School Street, Gorham, ME  04038.

Option 2 - PDF File

Instructions:

• Download the PDF version to your computer and print a copy of the report.
• Fill out the document completely.
• Mail a copy of the completed report to: CEPARE, c/o Ida Batista, 99 School Street, Gorham, ME  04038.
