We help policymakers and service providers implement effective integrated models of care for those with complex care needs.  We help design and implement improvements to long term services and supports, with a focus on helping people live healthy, active, secure and engaged lives, and promoting the sustainability and cost-effectiveness of public programs.

Leadership and Engagement

Leadership Stakeholder Engagement

We help state leaders and program managers design and implement large scale systems change initiatives.  Because policies and programs are strengthened when informed by the expertise and experience of both those receiving and delivering services, we help state leaders structure stakeholder engagement to leverage that experience and expertise in policy and program design and implementation. 

Integrated Care

Quality and Performance Measurement

We work with states to analyze, evaluate and report on key quality measures, such as cost and health outcomes, across behavioral health and chronic disease services. We provide Maine’s Office of MaineCare Services quarterly program monitoring and management reports. These quarterly reports provide ongoing analysis of health home progress and support program refinement, improved decision making and program management.

We offer technical assistance to support quality, actionable measurement and data collection designed to help inform decisions, monitor ongoing programmatic goals, refine program design and assess outcomes.

Supporting Accountable Care

We design and develop customized tools to support value-based purchasing. 

In Maine we have provided analytic, technical and operational support to Maine’s value-based purchasing initiatives including identification of frequent emergency department users; profiling high cost users; and identifying eligible health home members. 

We also developed and now maintain Maine’s Health Home Enrollment System, a secure web-based portal that allows health homes, community care teams and behavioral health homes to manage their panel enrollment, attest to providing health home services for payment, and review claims-based quality measures and utilization data.  The portal has now been expanded to support Maine’s accountable communities.

Behavioral Health Homes

As Maine moves toward an integrated approach to healthcare we have led the development of a competency-based approach to educating health home coordinators serving children and adults with serious mental illness. We developed and maintain a growing web-based resource center for health home coordinators looking to expand their knowledge of medical treatments, conditions, and terms.

Integrated Care for Persons Dually Eligible for Medicare and Medicaid

We are part of an evaluation team, led by RTI International, evaluating state demonstrations to integrate care for persons dually eligible for Medicare and Medicaid services.  These demonstrations are conducted through the Centers for Medicare & Medicaid Services Financial Alignment Initiative.

Long Term Services and Supports

Home and Community-Based Services (HCBS)

We are helping Maine reform its long term services and support programs to improve its compliance with new federal rules governing HCBS.  Our work involves developing data collection tools for assessing provider compliance and developing strategies for assuring ongoing compliance.  We are also providing support to the Maine Department of Health and Human Services as it solicits stakeholder input on implementation. 

Money Follows the Person (MFP)

We have provided technical assistance to the Maine Department of Health and Human Services in the design and implementation of its MFP demonstration.  We have also helped to integrate the MFP demonstration with Maine’s assessment, service authorization and payment system; developed quality management reports for monitoring ongoing operation and reporting; and provided support in developing training for transition coordinators.
