Exit Interviews

Please share with other faculty and staff that exit interviews are being provided for those who end employment with USM. With the employee's permission, the exit interview information will be used to provide feedback to supervisors regarding the employee's experiences here at USM; also, the information is used in planning for appropriate staff development and training programs. A written questionnaire will be provided or, upon request, an individual phone or personal interview. If you, or anyone on your staff is planning to resign, retire, etc., in the near future, please contact Claire Hassler at 780-5175 (Gorham campus) to schedule an appointment or to request a questionnaire.

The following are other things to be taken care of when someone is ending USM employment:

Telecommunications

Delete long distance authorization code; cancel calling card; update directory information.

Computing

Inform IT User Services as to employee's last day for adjustment of computer access rights.

University Property

Return property checked out from Property Management for home use. Turn in other items such as keys, corporate credit cards, etc.

Health Insurance Continuation

The insurance carrier will mail information automatically for anyone enrolled in University Health Insurance to provide the option to continue coverage at the group rate. If in need of details now for planning purposes, call the Benefits Office 780-5218.

Retirement Plans

Your last day of work will be your final day of contributions to TIAA-CREF or any of our other retirement plans. If you wish to change any of your contributions for your remaining salary you must submit the necessary forms to Benefits prior to when payroll runs. Please call the Benefits Office at 780-5218 for further information regarding requirements and restrictions.

Employee Data Change Form

Submit to Payroll showing termination date for final pay calculations.

As soon as possible, and no later than two weeks prior to leaving, provide a memo or letter indicating you are resigning/terminating your employment at USM. Send to Claire Hassler, McLellan House, Gorham campus.

Other possible items

Obtain letter of reference and/or recommendation; discard USM parking sticker, leave forwarding address.
