Recital forms
Student Recital Date Reservation Application - This will place a hold on this date for your recital, but will not be confirmed until the permit application is signed by your applied music instructor and filed with the Music Operations Director.
Student Recital Permit Application - requires signatures of your applied music instructor and other performers in your program.
Student Recital Program Template - 2011-2012 - please plan to submit this at least 6 weeks prior to your recital date.
Work study forms
Music Operations student work agreement:
1. First sign up for Work Study with the Music Box Office
2. Go to the Music Operations Blackboard group to find the Master Schedule. Open positions are highlighted in yellow. Fill in your name for the dates you agree to work, and e-mail the spreadsheet to larsenault@usm.maine.edu.
