Complete this form to request a copy of your faculty letter. Fields with a red * are required. Letter requests will be processed within 5 business days and letters will be delivered to your maine.edu e-mail address. Students who desire paper copies of their letters may print their letters free of charge at the DSC, 242 Luther Bonney Hall, Portland Campus. Students located at the Gorham and Lewiston Auburn campuses may obtain a paper copy of their letter from their respective Student Success office by e-mailing a request to firstname.lastname@example.org.
It is recommended that students make an appointment to meet with each of their professors as soon as possible at the beginning of the semester. Students should take their letters to these meetings. At the meeting, the following topics should be discussed:
- The impact of your disability
- The accommodations you are eligible to receive and how they apply to the course format
- The responsibilities you and your professor will each share in order to facilitate the outlined accommodations.
