Accommodations Request Form

Complete this form to request a copy of your faculty letter.  Fields with a red * are required.  Letter requests will be processed within 5 business days and letters will be delivered to your maine.edu e-mail address.  Students who desire paper copies of their letters may print their letters free of charge at the DSC, 242 Luther Bonney Hall, Portland Campus.  Students located at the Gorham and Lewiston Auburn campuses may obtain a paper copy of their letter from their respective Student Success office by e-mailing a request to dsc-usm@maine.edu.

It is recommended that students make an appointment to meet with each of their professors as soon as possible at the beginning of the semester.  Students should take their letters to these meetings.  At the meeting, the following topics should be discussed:

  1. The impact of your disability
  2. The accommodations you are eligible to receive and how they apply to the course format
  3. The responsibilities you and your professor will each share in order to facilitate the outlined accommodations.

Letter requests will be processed the week preceding the start of each semester.  Letter requests received after the start of the semester will be processed within 5 business days.  All letters will be delivered to your maine.edu e-mail address.  Students wanting paper copies of their letters may print their letters free of charge at the DSC, 242 Luther Bonney Hall, Portland Campus.  Students located at the Gorham and Lewiston Auburn campuses may obtain a paper copy of their letter from their respective Student Success office by e-mailing a request to dsc-usm@maine.edu.

Reminder:  You signed a general release at your initial intake appointment granting permission for the DSC at USM to disclose and exchange information regarding your disability, academic performance, residential status and accommodations with your faculty, advisor(s) and other essential USM personnel to facilitate access and to reinforce your success at USM. 

You also may have granted permission for the DSC to disclose and receive information in your DSC file to your parents.  Consents to release information remain current and active until you meet one of the following criteria: graduation or program completion; continuous unenrolled status for a period of 5 years; submission of a written request to revoke release.  Written revocation of release can be sent to Joanne Benica, PO Box 9300, 242 Luther Bonney Hall, Portland ME 04104 or e-mailed to dsc-usm@maine.edu.  Additional questions and concerns should be directed to dsc-usm@maine.edu.
