Request for Reproduction and/or Permission to Use

Conditions Governing Reproduction, Publication or Use of Materials in the University of Southern Maine’s Special Collections

  1. Decision to permit use of items from Special Collections will be made in compliance with its procedures, and with due regard for the nature and condition of the item. See our Reproduction Policy.
  2. Credit line must follow this formula: Name of individual collection, Name of meta-collection, Special Collections or Jean Byers Sampson Center for Diversity in Maine, University of Southern Maine Libraries. Example Credit Line: Am Chofshi Archives, LGBTQ+ Collection, Jean Byers Sampson Center for Diversity in Maine, University of Southern Maine Libraries
  3. Permission will be granted for a single use in a single edition of a single publication or a single exhibition. Additional language editions and new editions or exhibitions will be considered upon application. The University of Southern Maine Libraries will not grant exclusive rights to the use of materials in its Special Collections to any party or parties, and does not surrender its own rights to publish the material or to grant permission to others to use the material.
  4. If a reproduction of a photograph, other graphic image, or object from the Special Collections is cropped in any way, the caption must include the word "detail". The image may not be changed in any other way.
  5. Applicant assumes full and complete responsibility for any infringement of copyright or literary right which may grow out of his/her use or publication of the material.
  6. Applicants are responsible for reproduction costs and use fees. Please refer to the Reproduction Policy for current rates. 
  7. Applicant will provide University Libraries with one complimentary copy of the publication in which reproduced images appear.
  8. Special Collections reserves the right to deny future access to its resources for failure to meet any of these conditions.
I hereby apply for permission to reproduce or use the following items:
For Manuscripts: Call number, Box Number, Folder Number, Description (Ex. LGMS5, Box 1, Folder 5, Letter dated 4/7/18)
For Books: title, call number page number(s) or other identifier. (Ex. Creating Portland: History and Place in Northern New England, F29.P9 C73 2005, Pages 45-57)

(Include address or URL for an exhibition)
