NEW!   Our Service Request Form has been enhanced to provide improved customer service. Customers will receive email confirmation & updates when the form is submitted, and as work progresses.

  • Phone feature changes (e.g. ring tone, hunting/forwarding, call pick-up)

Please allow up to 10 business days for completion of service requests.

PLEASE NOTE: To report telephone/voicemail TROUBLES, please call 4029. The HelpDesk staff will create a trouble ticket and forward it to Telecommunications. For after hour troubles, leave your name, telephone number, location, and a brief description of the problem.



When completing the Service Request (RT) Form, be as detailed as possible when describing your request. Please include room numbers, user information and jack numbers when possible.

Hit the CREATE TICKET button located at the bottom of the page when form is completed.
A confirmation email will be sent once the form is submitted.
