• The first three forms (Camp Policies, ACA Health Update, and Photo/Media Release) are required, filled out on our registration software, upon signing up for a camp. We’re providing them here for reference.
  • The “Allergy/Emergency Medical Plan” is not a required form, but please use this if your child has serious food or environmental allergies. It helps our staff plan for difficult times. This can also be uploaded through the registration software.
